Delaware Care Collaboration saved $5 million in health care costs.
The collaboration is an Accountable Care Organization (ACO) and member of Trinity Health Mid-Atlantic, whose partners include Saint Francis Healthcare and the Medical Society of Delaware and also a new affiliation with the New Castle County Physicians Organization, saved $5 million in health care costs for Medicare patients.
The ACO also scored well in quality measures (92.17 percent), according to data released by the Centers for Medicare and & Medicaid in its 2019 Performance Results and Quality Report.
The collaboration emphasizes prevention, active management of chronic disease, and delivery of the highest quality of care so patients get the care they need before they are in crisis, for example, in areas such as heart care, pulmonology, wound care, physical and occupational therapy. A multidisciplinary team of social workers, community health workers, care managers, and provider relations professionals work together to support DCC participants.
“Delaware Care Collaboration’s new partnership with the New Castle County Physicians Organization will allow us to expand participation in and engagement with DCC for Delaware-based providers. Participating providers and their patients will benefit from our multidisciplinary support for practices, and planned investments in chronic care management and care management,” said Daniel L. Bair, regional executive director of Trinity Health Mid-Atlantic Clinically Integrated Network.
